Webinar Request Form Instructions:
- Choose the time and date you wish to have the webinar (Alternate Time is optional)
- Provide a title, brief description, and target audience for the webinar.
- Fill out requestor information (this is the person filling out the form). If requesting on behalf of another staff member, click the appropriate box. This will bring down a drop down menu for the additional contact. Complete this information.
- Click the Yes button to have participants preregister. Preregistration means that participants will be asked to register through the LTLTI Event Calendar as they would a normal conference.
- If you want to have incorporated content, select the Yes button next to the Content category. Select the number of files you are going to use and a brief description (title will suffice) of the files. If you do not know how many files you'll have at first, just select Other and describe them as "Additional files to be provided"
- If you plan on having a series of webinars with the same or similar topic, click Yes under the Recurring Series category. Complete the dates and titles of each webinar as needed. This is so you will not need to submit a separate form for each webinar in a series.
- Review the terms of the webinar request.
Once the form is receive it will be reviewed and you will be contacted with any further information needed. Please make sure any files you plan to use are emailed to P4A no later than the day before the webinar. Please keep in mind that a webinar tentative until confirmed by P4A-LTLTI as we may need additional information from the requestor to complete the scheduling process. Confirmation is received via an MS Outlook Appointment to be accepted by the requestor.
The P4A/LTLTI Webinar Request Form needs to be completed in full prior to the submission for the request of webinar services. Webinar capacity is, by default, limited to 96 different connections. If you anticipate have more than 96 connections to a webinar, P4A/LTLTI must be notified a minimum of 72 hours prior to the scheduled date of requested webinar in order to accommodate additional connections. All webinars are recorded unless otherwise requested. P4A/LTLTI will store and host all recordings of webinars. Requestor will receive follow up reports and recording links following the completion of each webinar. The Requestor will also receive a link to a free online survey for webinar participants to gauge the quality and utility of the presentation, in accordance with the requests of the P4A/LTLI and PA. Department of Aging & Office of Long Term Living. Submission of a Webinar Request Form does not guarantee the requested time and date is available. The Requestor for the webinar or series of webinars w ill receive notification via email once P4A/LTLTI confirms that all dates, times, and information needed for scheduling the webinar have been received. P4A/LTLTI will need to receive an attendance sheet for each location in which multiple staff participate in a webinar from a single location in accordance with records and archives. This attendance sheet will need to be received no later than one (1) week after the completion date of each webinar.
