Coleman Care Transitions Intervention Training

  • The Long Term Living Training Institute (LTLTI) is pleased to announce that the Coleman Care Transitions Intervention Training will be held at the Hilton Harrisburg, One N. Second St., Harrisburg, PA, on June 26—27, 2012. Under the leadership of Dr. Eric Coleman, the aim of the Care Transitions Program® is to encourage patients to take more control of their healthcare as well as improve patient transitions from one care setting to another.


    All participants listed below must be pre-approved for participation by the Care Transitions Program® by the Coleman Care Transitions Team. The training is limited to 35 people. The $500 per person registration includes—the cost of training, continental breakfasts, breaks and lunches. Lodging and evening meals are not included.
